Aged Care & Community Assistance
The participant's name and identifying details have been changed to protect her privacy.
Margaret wanted to remain in the home and neighbourhood she loved. After a change in mobility made some daily activities difficult, she and her daughter looked for dependable support that respected Margaret's choices.
The challenge
Margaret was managing well emotionally but needed help with shopping, meal preparation, transport and some household tasks. Her daughter lived across town and worried about safety, missed appointments and increasing social isolation.
Our approach
Margaret chose the type and timing of each support. We paired her with familiar workers, planned safer ways to complete tasks and reintroduced community outings at a pace that felt comfortable.
- Shopping and meal preparation
- Transport to appointments
- Safe support around the home
- Social and community outings
The outcome
Margaret continues to live at home and is again attending a local social group. A reliable weekly rhythm has reduced stress for both Margaret and her daughter, while keeping Margaret in control of decisions about her everyday life.
“I still decide how I spend my day. Having the right help means I can keep doing the things that matter to me.”
